Overview:

The Office of Rural Health Policy's Rural Health Care Services Outreach Grant Program encourages the development of new and innovative health care delivery systems in rural communities that lack essential health care services. The emphasis of this grant program is on service delivery through collaboration, requiring the lead applicant organization to form a consortium with at least two additional partners. The community being served must be involved in the development and ongoing operations of the program, to appropriately address the needs of the population. Programs funded have varied greatly and have brought care that would not otherwise have been available to at least two million rural citizens across the country. Through consortia of local providers and others, rural communities have managed to provide services such as hospice, dental care for children, and prenatal care in many remote areas.

The Outreach program supports projects that demonstrate creative or effective models of outreach and service delivery in rural communities. Applicants may propose projects to address the needs of a wide range of population groups including, but not limited to, low-income populations, the elderly, pregnant women, infants, adolescents, rural minority populations and rural populations with special health care needs. All projects should be responsive to any unique cultural, social, religious, and linguistic needs of the target population.

Eligibility:

Eligible lead applicants are rural, nonprofit private or public entities that represent a consortium of three or more organizations that deliver health care services in rural areas. Only the applicant organization is required to be a rural private or public not-for-profit entity. Faith-based and community-based organizations are eligible to apply.

Cost Sharing/Matching:

This program does not require cost sharing. However, consortium members are encouraged to support the project, either with cash or in-kind, to demonstrate commitment to the project.

Application Submitting Process:

The application and submission process has changed significantly. HRSA is requiring applicants for this funding opportunity to apply electronically through Grants.gov. All applicants must submit in this manner unless the applicant is granted a written exemption from this requirement in advance by the Director of HRSA’s Division of Grants Policy. Grantees must request an exemption in writing from DGPWaivers@hrsa.gov, and provide details as to why they are technologically unable to submit electronically though the Grants.gov portal. Make sure you specify the announcement number for which you are seeking relief. As indicated in this guidance, HRSA and its Grants Application Center (GAC) will only accept paper applications from applicants that received prior written approval.
