Small Health Care Provider Quality Improvement Grant Program

Program Purpose:

The purpose of the Office of Rural Health Policy's Small Health Care Provider Quality Improvement Grant Program (SHCPQI) is to assist rural providers with the implementation of quality improvement strategies, while improving patient care and chronic disease outcomes. Improving the quality of chronic disease management in ambulatory care settings can improve health indicators and decrease emergency room visits and admissions to hospitals SHCPQI will focus on quality improvement for chronic diseases, i.e., diabetes mellitus and cardiovascular disease.

Overview:

The grant program described in this document, the SHCPQI is authorized by Section 330A(g), Title III of the Public Health Service Act, as amended. This program is available to support rural public, rural non-profit, or other providers of healthcare services, such as a critical access hospital or rural health clinic.

The ultimate goal of SHCPQI is to improve patient health outcomes through enhanced chronic disease management in rural health care settings by:

1) Utilizing a patient registry system

2) Tracking and reporting specific health indicators using nationally accepted clinical measures

3) Assessing the need for quality improvement and developing additional performance measures

4) Participating in technical assistance through peer learning workshops with fellow SHCPQI Program grantees facilitated by a quality improvement specialist.
